Membership

To apply for membership, please:

1. Complete the Membership Application form, leaving the Proposer & Seconder part vacant. We will complete that requirement.
2. Complete the Members Details form. Please complete ALL the details, as we need to have your details for future contact.
3. Don't forget the $10. (Most put in a $10 note. Saves the card processing)
4. Mail the forms back to: Pharmacy Cricket Inc, 25 Annesley Locked Bag 2500, Bowral NSW 2576

Once the membership forms are received your Membership Certificate will be issued.
